Overview
We are seeking a dedicated and highly skilled Primary Care Physician to provide comprehensive healthcare services within a multidisciplinary medical team. The ideal candidate will possess a strong foundation in internal medicine and family medicine, with a commitment to delivering patient-centered care. This role involves managing a diverse patient population, conducting thorough assessments, and developing personalized treatment plans to promote health and well-being. The Primary Care Physician will play a vital role in preventive care, chronic disease management, and health education, ensuring optimal health outcomes for patients across all age groups.
Duties
- Conduct detailed medical history reviews and comprehensive physical examinations to assess patient health status.
- Develop, implement, and monitor individualized care plans tailored to each patient's needs, including preventive measures and chronic disease management.
- Utilize electronic health record (EHR--Elation) systems effectively for documentation, order entry, and patient tracking.
- Provide patient education on health promotion, lifestyle modifications, medication adherence, and disease prevention strategies.
- Coordinate with specialists and ancillary services such as radiology, laboratory testing (including medical imaging), and pharmacy to ensure integrated patient care.
- Manage acute illnesses and minor injuries while determining when specialist referral or hospital admission is necessary.
- Ensure compliance with HIPAA regulations and maintain accurate medical records in accordance with legal standards.
- Participate in clinical research or quality improvement initiatives aimed at enhancing patient outcomes and service delivery.
Requirements
- Medical degree (MD or DO) from an accredited institution with valid licensure to practice medicine.
- Board certification or eligibility in family medicine or internal medicine is preferred.
- Proven experience in outpatient primary care settings, including clinic experience and hospital medicine.
- Proficiency in EMR/EHR systems such as Elation or similar platforms for documentation and patient management.
- Strong knowledge of medical terminology, physiology, pharmacology, and diagnostic evaluation techniques.
- Familiarity with procedures such as suturing, basic airway management, and IV infusion is desirable.
- Excellent communication skills with the ability to build rapport with diverse patient populations.
- Ability to handle complex cases involving chronic conditions like diabetes, hypertension, cardiology issues, endocrinology concerns, and mental health conditions such as PTSD or behavioral health needs.
- Experience working within HIPAA guidelines ensuring patient confidentiality and data security.
- Commitment to ongoing professional development through participation in clinical research or continuing education programs related to primary care practices.
