Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity.
- Support patients with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques to promote safety.
- Monitor vital signs, observe patient conditions, and report any changes or concerns to healthcare professionals promptly.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Medication reminders.
- Provide transportation assistance for medical appointments, community outings, or errands, ensuring safe and reliable travel.
- Maintain detailed documentation of care provided, patient progress, and adherence to prescribed treatment plans while respecting confidentiality standards.
Skills
- Proven experience working with individuals with disabilities, neurodevelopmental disorders, or developmental disabilities in home or community settings.
- Strong knowledge of dementia care, Alzheimer’s care, memory care, and hospice care practices.
- Familiarity with manual handling techniques such as heavy lifting and the use of assistive devices like Hoyer lifts.
- Excellent communication skills for engaging with clients and their families to ensure clarity about care routines and progress updates.
- Ability to perform light housekeeping tasks including cleaning and maintaining a safe environment for clients.
- Knowledge of HIPAA regulations and experience working within home health or long-term care environments is highly desirable.

Pay: $12.00 - $14.00 per hour
