Greenfield Rehabilitation Agency is expanding and seeking passionate Physical Therapists (PTs) to join our exceptional team at our senior campuses in Lancaster, while also supporting communities in Prairie du Chien. At Greenfield Rehab, we provide functional, effective, and rewarding physical therapy across skilled nursing facilities, senior communities, assisted living environments, and in-home care.
This is a unique opportunity to combine hands-on clinical care with flexible scheduling—whether you’re seeking full-time, part-time, or PRN work—while making a real difference in the lives of older adults across multiple communities. Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance
Identify barriers to functional independence and design individualized physical therapy programs
Address challenges related to mobility, posture, gait, and injury or illness recovery
Provide interventions to help residents regain strength, prevent falls, and maintain independence
Support residents in daily functional tasks, recommending adaptive equipment as needed
Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function
Assess living spaces to recommend modifications for safety and accessibility
Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention
Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care
Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies
