As a Service Navigator for Massachusetts Department of Developmental Services (DDS) funded programs the desired person for this position would provide person- and family-centered case management, budget administration, and resource coordination. Key responsibilities include developing individualized support plans, managing flexible program budgets, and connecting families to community resources.
Core Responsibilities
- Individualized Planning: Partner with individuals and families to identify strengths, needs, and goals, and develop actionable support or family navigation plans.
- Budget & Program Management: Administer approved flexible funding budgets (such as for DESE/DDS or Agency with Choice programs), track spending, and submit financial paperwork to DDS.
- Service Coordination: Assist in finding, onboarding, and scheduling approved providers, such as skills trainers, respite workers, and therapists.
- Collaboration & Advocacy: Maintain active communication with DDS Service Coordinators, school districts, and generic community resources.
- Documentation & Compliance: Maintain case notes, contact logs, goal progress updates, and mid-year/end-of-year reports in line with DDS regulations.
Qualifications & Requirements
- Education: High school diploma/GED required; Associate's or Bachelor’s degree in human services, special education, or a related field is preferred.
- Experience: Minimum of 2 years of human services experience working with individuals with intellectual disabilities, autism, or developmental/complex support needs preferred
- Travel & Schedule: Outreach-based role requiring regular home visits, flexible hours, a valid driver’s license, and a reliable insured vehicle.
- Certifications: Ability to pass background checks (CORI/fingerprinting) and maintain safety/medical certifications like CPR and First Aid.
Pay: From $20.00 per hour
