**Some travel is involved. Meetings can occur in Richmond, Virginia Beach, Suffolk, Norfolk, Newport News, Hampton, Chesapeake, etc.**
General Responsibilities: The individual will be responsible for providing QDDP Services for adults and children with intellectual and developmental disabilities.
The QDDP assumes responsibility for the following roles to include:
- Conducting and approving assessments
- Prescreening, admitting, and discharge planning
- Intake screening and associated paperwork
- Attend Intake, SIS, and Annual meetings
- Overseeing the development and implementation of Individualized Service Plans (Part V Initial and Annual)
- Writing reports such as Quarterly Service Reviews
- Reviewing documentation on the individuals
- Coordinating services with Support coordinator to include scheduling meetings
Knowledge, Skills, and Abilities:
- Experience managing services for adults and children with intellectual, developmental, physical, and mental disabilities
- Experienced in personnel management, HIPPA and records management, developing and implementing Individualized Service Plans, conducting, and approving assessments, prescreening, and discharge planning
- Familiarity with regulations governed by DBHDS and Offices of Human Rights, Medicaid, Department of Labor, OSHA, Community Service Boards, Case Management Services, First Aid, and CPR
- Effective written and oral communication skills
- Strong documentation skills
- Knowledge of behavioral plans
Qualifications:
- A Bachelor’s degree plus one year of experience and supervisory skills working with the intellectually and developmentally disabled population served
- Must have a valid Virginia driver’s license
- Must have one (1) year of working with the intellectually and developmentally disabled population
- Certifications are preferred for Therapeutic Options, First Aid, and CPR. Will provide this training if necessary.
