Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment such as Hoyer lifts.
- Monitor and record vital signs accurately while observing patient conditions to detect changes or concerns early.
- Administer medications following prescribed protocols, ensuring compliance with home healthcare regulations and maintaining precise medical documentation.
- Provide specialized dementia and Alzheimer's care, including memory support strategies and maintaining a safe environment for residents with cognitive impairments.
- Support end-of-life care with compassion, respecting patient wishes and collaborating with family members and healthcare teams to deliver dignified hospice services.
- Maintain patient confidentiality by adhering to HIPAA regulations and safeguarding medical records diligently.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe, hygienic living environment for clients.
- Document medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately and promptly.
- Support patient transfer methods utilizing proper techniques to ensure safety for both patients and caregivers during mobility assistance.
- Engage in ongoing training in first aid, CPR, basic life support (BLS), and other certifications to enhance patient safety and emergency response capabilities.
Skills
- Strong knowledge of medical terminology relevant to geriatric and long-term care settings.
- Experience working with individuals with developmental disabilities, dementia, or geriatrics in assisted living or nursing home environments.
- Proficiency in using EMR/EHR systems for documentation and medical records management.
- Excellent patient observation skills to identify changes in condition or behavior promptly.
- Ability to communicate effectively with patients, families, and healthcare teams while maintaining professionalism and empathy.
- Familiarity with home health care assistance protocols, including medication administration and vital signs monitoring.
- Knowledge of home & community care regulations to ensure compliance during service delivery.
- Physical stamina for heavy lifting, patient transfers, and long shifts; experience operating Hoyer lifts is a plus.
- Ability to work independently or as part of a team in live-in employment or shift-based roles within home health agencies or hospice settings.
- Commitment to maintaining patient confidentiality under HIPAA guidelines while providing compassionate caregiving services.
