MDS Coordinator
THRYVE Healthcare, a leading healthcare organization, is seeking a highly skilled and dedicated MDS Coordinator to join our team. As an integral member of our clinical staff, the MDS Coordinator will play a critical role in ensuring the highest quality care for our residents while maintaining compliance with industry regulations and standards.
Responsibilities:
Coordinate and conduct resident assessments, including the Minimum Data Set (MDS) 3.0 assessment, to ensure accurate and timely completion of assessments
Develop, implement, and maintain resident care plans in collaboration with the interdisciplinary team
Utilize electronic medical records (EMRs) to document and track resident information and assessments
Participate in Care Plan Meetings and other interdisciplinary team meetings to ensure resident-centered care
Collaborate with other departments, such as Nursing, Therapy, and Social Services, to develop and implement individualized care plans
Identify and report potential quality of care issues and opportunities for improvement to the Director of Nursing and other stakeholders
Requirements:
2 years of experience as an MDS Coordinator (or equivalent experience in long-term care or acute care setting)
Strong knowledge of MDS 3.0 assessment and care planning processes
Excellent communication and interpersonal skills, with the ability to work effectively with interdisciplinary team members
Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines
Ability to maintain confidentiality and handle sensitive information with discretion
