We are seeking a highly motivated, qualified Illinois licensed Advanced Practice Provider to join our team. The Advanced Practice Provider is responsible for coordinating patient care to ensure high-quality, cost-effective, and patient-centered outcomes. This role requires travel throughout central Illinois and involves assessment, care planning, utilization management, patient advocacy, and collaboration to support optimal health outcomes.

- Current NP or PA license in IL
- National certification (ANCC, AANP, NCCPA, as applicable)
- DEA license (or ability to obtain
- Experience in skilled nursing, long-term care, geriatrics, rehabilitation, or post-acute care strongly preferred
- Knowledge of CMS regulations, F-tags, and survey processes is a plus
- Knowledge of Medicare Part A, managed care, and skilled-nursing facility reimbursement processes
- Excellent clinical assessment and problem-solving skills
- Strong communication skills with residents, families, providers, and payer representatives
- Must be charismatic, energetic, and people-oriented
- Good working knowledge of computers
