Overview:
Are you a dedicated Licensed Vocational Nurse (LVN) looking to make a meaningful impact on patients' lives? We are seeking compassionate and skilled LVNs to join our team at Care Connection Home Health Agency where your expertise and caring nature will be valued and supported.
Responsibilities:
- Provide high-quality nursing care to patients under the supervision of RNs and physicians.
- Administer medications and treatments as prescribed.
- Monitor patients’ vital signs and report any changes to the healthcare team.
- Assist in developing and implementing patient care plans.
- Maintain accurate patient records and documentation.
- Ensure a clean and safe environment for patients and staff.
Qualifications:
- Current LVN license in California.
- CPR/BLS certification
- Ability to build rapport with patients.
- Compassionate and caring demeanor
- Excellent communication and interpersonal skills.
- Strong attention to detail and organizational abilities.
- Ability to work effectively in a team-oriented environment.
- Previous experience in a healthcare setting is preferred but not required.
