We are seeking a dedicated Registered Nurse to deliver high-quality, one-on-one care with flexibility, autonomy, competitive compensation, and comprehensive benefits.

- Conduct patient admissions and develop individualized Plans of Care
- Obtain medical histories and perform physical assessments
- Monitor patient status, vital signs, and clinical progress in the home setting
- Deliver skilled nursing care in compliance with HIPAA and agency standards
- Evaluate patient outcomes and adjust care plans as needed
- Collaborate with the interdisciplinary team to ensure comprehensive care
- Coordinate services with LPNs, HHAs, and therapy providers
- Complete accurate documentation and submit clinical notes in a timely manner
- Current Registered Nurse (RN) license in Connecticut
- OASIS and WellSky (Kinnser) experience preferred
- Medical-surgical and wound care experience preferred
- Physical exam within the last 12 months
- TB test within 12 months and current CPR certification
- Valid driver’s license and reliable transportation
- Ability to attend a 1–3 day orientation at our Stamford office
