Main Office location: Westwood, MA

Service Area: This position requires travel to our multiple locations in New England, Pennsylvania and Virginia.

Base Salary Range: $95,000 - $105,000
Responsibilities:
-
Assist in creating and implementing education curriculum with Clinical Education department.
-
Demonstrates techniques and best practices for trainees.
-
Assists in creating and implementing policies and procedures for all clinicians.
-
Initiate appropriate preventive and rehabilitative nursing procedures.
-
Administer medications and treatments as prescribed by physicians.
-
Conduct initial assessments of patients and families to determine home care needs.
-
Perform thorough physical assessments and gather comprehensive medical histories.
-
Develop and revise individualized plans of care based on patient status and evolving needs.
-
Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
-
Responsively address inquiries and concerns from patients, caregivers, and medical teams.
-
Provide counseling and support to patients and caregivers before, during, and after treatment.
-
Supervise home health aides as required.

Identify discharge planning needs and proactively implement them as part of the care plan development.
Qualifications:
-
Preferably 2+ years of nursing experience, with a focus on geriatrics.

Nursing degree from an accredited institution.
-
Excellent bedside manners and exceptional communication skills.
-
Working knowledge of healthcare laws and regulations.
-
Maintain current BLS CPR certification.

Possess a valid driver's license and own a reliable, insured vehicle.
