The opportunity
We are seeking an experienced, hands-on operations leader to run the day-to-day business across our home care and home health locations serving the Florida Panhandle (Bay County, Okaloosa County, Pensacola, and Tallahassee) and South Alabama (Mobile and Spring Hill). This is a rare opportunity to step in as the operational leader of an established, mission-driven business with a strong pediatric Medicaid foundation — and to be the driving force behind its next chapter: diversifying into private pay, VA, specialty infusion, and commercial national accounts.
You will lead a team of roughly 15 across both states, own P&L performance, and be the connective tissue between clinical operations, HR/recruiting, compliance, and business development. This role is ideal for someone who has thrived in a small or founder-led business, knows how to build systems where few exist, and is energized by the challenge of growing new payor relationships without losing sight of the patients and families who depend on the core Medicaid pediatric program.

- Multi-state operation covering the Florida Panhandle (Bay County to Okaloosa, Pensacola, Tallahassee) and South Alabama (Mobile, Spring Hill).
- Approximately 85% of current revenue is Medicaid, concentrated in pediatric home care/home health cases.
- Team of approximately 15 employees across clinical, scheduling, intake, and administrative functions.
- Strategic priority: diversify the payor mix into private pay, VA (Veterans Affairs) home care, specialty infusion, and commercial insurance / national accounts.
What you will own
Operational leadership
- Serve as the single point of accountability for daily operations across both states — staffing, scheduling, service delivery, quality, and compliance.
- Build and strengthen the operating rhythm of the business: KPIs, scorecards, staff meetings, and performance reviews.
- Ensure consistent, high-quality care delivery across all branch locations and case types, particularly the pediatric Medicaid population.
- Identify and resolve operational bottlenecks in intake, authorization, scheduling, and billing workflows.
Team leadership
- Directly lead and develop a team of approximately 15, including clinical and administrative staff, across two states.
- Recruit, hire, coach, and manage performance for branch-level leaders and staff.
- Build a culture of accountability, clinical excellence, and responsiveness to patients and families.
Payor mix diversification & growth
- Design and execute a strategy to grow private pay, VA, specialty infusion, and commercial/national account business alongside the existing Medicaid pediatric base.
- Build referral relationships with hospitals, physician groups, discharge planners, VA facilities, and specialty infusion providers/payors.
- Evaluate and stand up the operational, billing, and compliance infrastructure needed to support new payor types and case types.
- Track and report progress against payor mix diversification goals to ownership.
Compliance & risk
- Ensure ongoing compliance with Florida and Alabama home care/home health licensure, Medicaid, and applicable federal regulations.
- Partner with ownership and outside advisors (legal, HR, clinical) on compliance matters as they arise.
- Maintain audit-readiness across both states' regulatory frameworks.
Multi-site & multi-state presence
- Regularly travel across the Florida Panhandle and South Alabama service areas to be visible and available to teams, referral partners, and key accounts.
- For the first 3–6 months, spend significant time on the ground in-market (Panhandle and Mobile/Spring Hill) to build relationships, understand operations firsthand, and establish credibility with staff and referral sources.
What you bring
- Prior experience leading operations or serving as a general manager in a small or mid-sized business, ideally in a founder-led or entrepreneurial environment.
- Demonstrated experience leading and developing teams of similar size (10–25 people).
- Home care, home health, or broader healthcare services industry experience strongly preferred.
- Experience with Medicaid-funded services and/or exposure to private pay, VA, specialty infusion, or commercial payor relationships is a strong plus.
- Comfort operating with a hands-on, roll-up-your-sleeves style typical of small businesses — this is not a role for someone who needs a large existing infrastructure.
- Willingness and ability to travel regularly within the Panhandle/South Alabama footprint; if not currently based in the market, willingness to spend substantial time on-site, especially in the first 3–6 months.
- Strong business acumen: comfortable with P&L ownership, KPIs, and translating strategy into day-to-day execution.
- Excellent relationship-building skills for referral development with hospitals, physicians, VA facilities, and payors.
Location & travel
This role covers two service areas: the Florida Panhandle (Bay County, Okaloosa County, Pensacola, and Tallahassee) and South Alabama (Mobile and Spring Hill). Candidates based within or near this footprint are preferred. Candidates based elsewhere will be considered but should expect to spend significant time in-market, particularly during the first 3–6 months, to build relationships and establish operational rhythm on the ground.

Pay: $125,000.00 - $160,000.00 per year
