Job Overview
We are seeking a compassionate, energetic, and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our dedicated healthcare team. In this vital role, you will provide high-quality patient care in our including long-term care facilities. Your expertise will support patients with complex medical needs, including those with dementia, Alzheimer's disease, developmental disabilities, and acute or chronic conditions. Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications via injections, IV infusions, catheterization, and other methods following strict protocols for safety and accuracy.
- Manage complex medical procedures such as tube feeding, wound care.
- Document all patient interactions accurately using Electronic Health Record (EHR) system PCC while maintaining strict compliance with HIPAA and clinical confidentiality policies.
- Provide specialized care for patients with dementia, Alzheimer’s disease, memory impairments, or behavioral health conditions through tailored interventions.
- Assist with diagnostic evaluations such as specimen collection/processing and medical imaging coordination to support comprehensive patient care.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to ensure seamless transitions across care settings.
- Support emergency response efforts by applying first aid skills, airway management techniques, CPR certification, and acute pain management protocols when necessary.
- Educate patients and their families about health coaching, medication administration, nutrition plans, and long-term care regulations to foster informed decision-making.
Experience
- Valid RN or LPN license in good standing with relevant state licensing boards.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), skilled nursing facilities, hospice care settings, or outpatient clinics.
- Demonstrated expertise in critical care areas including ICU experience, emergency medicine (ER), trauma center procedures (Level II trauma experience), infectious disease care (ICD-10/ICD-9 coding), and internal medicine practices.
- Hands-on experience with EMR/EHR systems such as Point Click Care (PCC); familiarity with medical documentation standards is essential.
- Background in geriatrics or working with geriatric patients in long-term care or assisted living environments is highly preferred.
- Knowledge of advanced nursing techniques including IV infusion therapy, and DME (durable medical equipment).
- Experience working with individuals with disabilities or developmental challenges; familiarity with dementia/cognitive impairment care strategies is a plus.
- Strong understanding of anatomy knowledge, physiology principles, medication administration protocols—including injections—and infection control practices.
- Ability to perform physical examinations accurately while demonstrating excellent caregiver communication skills for effective patient education and family engagement.

Pay: $30.50 - $40.00 per hour
