Home Health Solutions Group is seeking compassionate, reliable, and experienced Certified Nursing Assistants (CNA's) to provide Continuous Care services for hospice patients in their homes or facilities. We staff nurses with Three (3) major hospices. This is an excellent opportunity for dedicated clinicians who are passionate about delivering high-quality, one-on-one patient care while enjoying a flexible work schedule and competitive compensation.
Our Continuous Care program supports patients experiencing periods of crisis, helping them remain comfortable and receive specialized nursing care in the comfort of their homes.

Responsibilities
- Provide non skilled nursing care to hospice patients requiring Continuous Care services.
- Monitor patient condition and promptly report changes to the hospice interdisciplinary team.
- Take vital signs
- Bathe, reposition and provide emotional support
- Maintain accurate and timely documentation.
- Educate and support patients and family members.
- Collaborate effectively with hospice staff, and caregivers.
Qualifications
- Active Florida CNA License for at least 1 yr
- Current CPR Certification
- Minimum one year with professional license
- Strong documentation skills
- Reliable transportation
- 11 to 12-hour shifts
- Day shifts
- Weekend availability
- Flexible scheduling
- Per diem
