Medcare Hospice is hiring a Patient Care Manager RN to lead and support our hospice clinical team. We are looking for a compassionate, organized, and experienced RN leader who is passionate about quality patient care, team development, and supporting patients and families during one of life’s most important stages.
What You’ll do as the Patient Care Manager, you will provide daily clinical leadership and oversight for the interdisciplinary hospice team. You will help ensure patients receive high quality, compassionate care while supporting staff, compliance, documentation, care coordination, and quality improvement efforts.
Key Responsibilities
- Lead and support the hospice interdisciplinary team.
- Coordinate patient care with nurses, aides, physicians, pharmacy, DME, admissions, and other departments.
- Monitor clinical documentation, quality indicators, and staff performance.
- Participate in IDG meetings, care planning, QAPI activities, and patient care conferences.
- Support staff training, coaching, and ride along visits.
- Help resolve patient and family concerns in a timely and compassionate manner.
- Participate in after hours on call and backup nursing duties as needed.
Requirements
- Current RN license in the state of employment.
- At least 1 year of RN management experience.
- Active BLS from the American Heart Association or American Red Cross.
- Valid driver’s license, auto insurance, and reliable transportation.
- Strong communication, leadership, and organizational skills.
- Hospice, home health, or palliative care experience preferred.
- Bilingual English and Spanish preferred.
