OB Coordinator

About the Role
The OB Coordinator plays a key role in supporting high‑quality obstetric care by combining clinical expertise, patient education, and program coordination. This position ensures continuity of care for pregnant and postpartum patients, improves maternal health outcomes, and strengthens collaboration across clinical and community partners.
Key Responsibilities
1. Patient Support & Care Coordination
- Provide direct support to pregnant and postpartum patients throughout their care journey.
- Conduct screenings for medical and obstetric history, OUD, and other risk factors.
- Track and monitor high‑risk patients; follow up on missed appointments and connect patients with recovery or support services.
- Coordinate with OB physicians and multidisciplinary teams to ensure timely follow‑up for mother and baby.
- Maintain communication with community partners and referral sources.
2. Education & Training
- Deliver or coordinate patient education on diabetes management, nutrition, glucose monitoring, and lifestyle counseling.
- Support patients with gestational or pre‑existing diabetes using evidence‑based teaching strategies.
- Reinforce self‑management skills and promote healthy pregnancy behaviors.
3. Program & Collaboration Development
- Develop and promote partnerships with community organizations.
- Enroll eligible patients in state and local maternal health programs.
- Connect patients with peer recovery coaching and other supportive services.
- Participate in bi‑weekly high‑risk provider meetings to review cases and coordinate care plans.
4. Administrative & Operational Duties
- Manage patient rosters, scheduling, and follow‑up on individualized care plans.
- Maintain accurate electronic health records (EHR) and generate reports for eligibility, program participation, and quality metrics.
- Prioritize tasks effectively, meet deadlines, and work independently or as part of a team.
- Support operational workflows to ensure efficient OB service delivery.
Qualifications/Education
Experience
- Minimum 2+ years of clinical experience.
- Obstetric experience strongly preferred.
Skills
- Strong knowledge of medical terminology and OB care.
- Proficiency with Microsoft Office and EHR systems.
- Excellent communication, problem‑solving, and organizational skills.
- Ability to work collaboratively with diverse patient populations and multidisciplinary teams.
