Duties
- Conduct detailed behavioral health intake assessments to evaluate patient needs, including mental health history, current symptoms, and social factors.
- Collaborate with multidisciplinary teams to develop personalized treatment plans and coordinate referrals for outpatient, inpatient, or post-acute care as needed.
- Manage patient records with precision, ensuring HIPAA compliance and accurate documentation of medical terminology, diagnostic evaluations, and discharge planning.
- Facilitate utilization management by reviewing medical records and supporting diagnostic evaluations to determine appropriate level of care.
- Assist in discharge planning by coordinating follow-up services and providing patients with necessary resources for ongoing support.
- Perform case management duties by monitoring patient progress and adjusting care plans accordingly within managed care frameworks.

- Skills
- Proven experience in hospital settings, especially within Level I or Level II trauma centers or acute care environments such as ICU or emergency medicine units.
- Strong knowledge of behavioral health, mental health counseling, addiction counseling, and social work principles.
- Familiarity with hospital medicine practices including inpatient and outpatient clinic operations.
- Experience with medical terminology, physiology knowledge, and diagnostic evaluation techniques relevant to behavioral health assessments.
- Skilled in patient assessment, discharge planning, medical records management, and utilization management processes.
- Ability to navigate HIPAA regulations diligently while handling sensitive patient information.
- Excellent communication skills for effective patient service delivery and case management coordination across multidisciplinary teams.
