Job Summary
Are you an RN looking for a rewarding, lower-stress clinical role with autonomy? Elora Home Care, LLC (pending RSA licensure) is seeking a passionate Registered Nurse (RN) to conduct in-home client assessments, develop personalized care plans, and provide clinical supervision to our future caregiving team. This is a flexible PRN/contract position perfect for nurses seeking supplemental income, case managers looking for field autonomy, or retired nurses wanting to maintain their clinical edge. There is no heavy wound care or intensive clinical procedures required.
Duties
- Initial Assessments: Visit new clients in their homes to evaluate physical, environmental, and psychosocial needs.
- Care Plan Development: Draft comprehensive, customized care plans that outline specific tasks for our in-home caregivers.
- 60-Day Re-Evaluations: Conduct routine, 60-day supervisory site visits to assess client progress, modify care plans as needed, and ensure compliance.
- Caregiver Supervision: Observe, evaluate, and mentor our in-home caregivers (CNAs/HHAs) during field visits to ensure high-quality care.
- Clinical Liaison: Act as the trusted clinical bridge between clients, their families, our caregivers, and agency leadership.
- Documentation: Complete electronic clinical notes and assessments accurately and promptly following each visit.
Experience
- License: Active, unencumbered Registered Nurse (RN) license in the State of Maryland (or compact state license).
- Experience: Minimum of 1 year of clinical RN experience. Home health, home care, hospice, or case management experience is highly preferred but not required.
- Transportation: Valid driver’s license, a reliable personal vehicle, and active auto insurance for local travel within Anne Arundel County and neighboring areas.
- Skills: Excellent communication, strong organizational abilities, and comfort working independently in diverse home environments.
- Tech-Savvy: Ability to comfortably utilize electronic health records (EHR) software on a smartphone or tablet for clinical documentation.
