Duties
- Assist clients with activities of daily living (ADLs), including personal care, grooming, dressing, and hygiene to promote dignity and independence.
- Monitor patients’ health by observing vital signs, administering medication, and documenting care plans in compliance with HIPAA regulations.
- Support clients during meal preparation, feeding, and nutrition management to ensure dietary needs are met.
- Operate patient transfer equipment such as Hoyer lifts and utilize proper manual handling techniques for safe patient transfers.
- Conduct patient observation to identify changes in condition or behavior, especially for individuals with dementia, Alzheimer's, or neurodevelopmental disorders.
- Maintain a clean and safe environment by performing light housekeeping duties such as cleaning client areas and ensuring infection control standards are upheld.
- Provide transportation assistance for appointments or community outings while ensuring safety protocols are followed.
- Collaborate with healthcare teams to implement care plans tailored to each individual’s needs and preferences.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred.
- Demonstrated experience in patient monitoring, medication administration, and vital signs measurement.
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and caring for individuals with developmental or intellectual disabilities is advantageous.
- Familiarity with patient transfer methods such as Hoyer lifts and resident’s <PERSON> (a placeholder for individual client names) management is beneficial.
- Experience working with individuals with autism or neurodevelopmental disorders enhances your ability to provide specialized support.
- Strong understanding of HIPAA compliance ensures confidentiality and privacy of client information.
- Ability to perform heavy lifting safely and assist with manual handling tasks as needed.
- Excellent communication skills for engaging effectively with clients, families, and healthcare providers.
