Duties
- Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor residents’ health by taking vital signs, observing for changes in condition, and reporting findings accurately while adhering to HIPAA privacy standards
- Administer medications and follow care plans under supervision to ensure safe and effective patient care
- Prepare nutritious meals in accordance with dietary restrictions and assist residents during meal times to promote independence and social interaction
- Support residents in engaging with community activities and social programs to enhance their quality of life
- Maintain a clean, safe living environment by performing light housekeeping duties including cleaning common areas and resident rooms
- Document care activities meticulously, update progress notes regularly, and collaborate with healthcare teams to develop or modify individualized care plans
Skills
- Proven experience working in assisted living, group homes, nursing homes, or home health environments with a focus on senior or disability care
- Knowledge of patient monitoring techniques including vital signs measurement and patient observation for safety concerns such as dementia or Alzheimer's disease
- Certification in CPR and First Aid is required; additional certifications like HIPAA compliance are highly valued
- Strong ability to assist with ADLs and provide compassionate patient care tailored to individual needs
- Experience with specialized care such as hospice, memory care, autism spectrum disorders, or working with individuals with developmental disabilities is preferred
- Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
- Excellent communication skills for collaborating with team members, residents, families, and healthcare professionals
