Job Title:
The Hospice On-Call Registered Nurse (RN) is responsible for after-hour on-call visits of patients. The Hospice On-Call RN will ensure that care and services are delivered appropriately and timely during after-hour and holiday coverage.
Job Duties:
- Demonstrates competency in skills required by the agency for patient care. Has received specialty program education and has completed satisfactory competency.
- Communicates and document any significant findings, problems, visits, changes in condition, level of care or environment to the clinical managers, medical director and other clinical staff members of the hospice care team.
- Assesses and reassesses physical, environmental and emotional factors to determine hospice needs.
- Utilizes the patient’s plan of care to maintain continuity of patient care by communication with appropriate staff.
- Answers all incoming calls in a manner appropriate to the nature of the call, complete admits, death, PRN visits and follow-up visits on patients admitted the day prior; as necessary.
- Responsible to assist with calling and getting report for referrals received after hours.
- Maintains flow of communication between clinical staff and other departments, for coordination of activities and services.
- Responsible for documenting and notifying Admin and/or Clinical Manager of any changes or follow up that is needed the following day.
- Administers medications and treatments in accordance with the patient’s plan of care.
- Works in cooperation with the family/caregiver and IDT to meet the emotional needs of the patient/family/caregiver.
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains physician orders as required.
- Submits all required documentation in an accurate and timely manner.
- Performs care to the set level of standards set by the company in order to provide patients with the highest level of pain and symptom management and a high-quality end-of-life care.
- Assists in providing patient/family with education on their plan of care, medications, equipment, etc. as needed.
- Performs other duties as assigned.
The RN will receive patient related calls, determine the needs of the caller and then respond in an appropriate manner by either visit or phone.
Qualifications:
- Must be a graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment
- Must possess a valid state driver’s license, automobile liability insurance, and have dependable transportation
- Minimum of one year of clinical experience is strongly preferred
- Previous home health or hospice experience is preferred
This position will be a full time position with 7 days on then 7 days off.
Salary will be discussed with management prior to an offer letter being sent to you.
