Home Health Clinical Manager (RN/PT/OT)

We are seeking an experienced and motivated Registered Nurse Clinical Manager to lead our Home Health team. This is an exciting opportunity for a strong clinical leader who is passionate about providing exceptional patient care while mentoring and supporting an outstanding team of clinicians.
As a Clinical Manager, you will play a key role in ensuring quality patient care, regulatory compliance, staff development, and efficient daily operations. Responsibilities
- Provide clinical oversight and leadership for home health nursing and therapy staff.
- Supervise, mentor, and support field clinicians to promote clinical excellence.
- Review and approve clinical documentation for accuracy, quality, and regulatory compliance.
- Ensure compliance with Medicare, Medicaid, VA, and payer guidelines.
- Assist with patient care coordination and interdisciplinary communication.
- Monitor quality outcomes and implement performance improvement initiatives.
- Participate in case conferences and clinical meetings.
- Assist with staff education, coaching, and onboarding.
- Support scheduling and staffing to ensure continuity of patient care.
- Collaborate with physicians, referral sources, and community partners.
- Promote a positive, supportive, and team-oriented work environment.
Qualifications
- Current Registered Nurse (RN) license.
- Home Health experience required.
- Previous leadership or supervisory experience preferred.
- Strong knowledge of Medicare Conditions of Participation and home health regulations.
- Excellent communication, organization, and critical thinking skills.
- Experience with electronic medical records (EMR) preferred.
- Ability to multitask and work collaboratively with an interdisciplinary team.

Pay: $80,000.00 - $95,000.00 per year
