Job Overview
We are seeking a passionate and dedicated Memory Care Coordinator to lead and enhance the quality of life for residents with memory impairments, including Alzheimer’s disease and other forms of dementia. In this vital role, you will oversee the development and implementation of personalized care plans, personalized activities, social services, and ensure a safe, engaging, and supportive environment. Your leadership will foster compassionate resident care while managing aspects to promote excellence in memory care services. Duties:
- Compassion and Empathy: A genuine desire to enhance the quality of life for individuals with memory impairments is essential.
- Dementia Knowledge: In-depth understanding of the stages of dementia, related behavioral changes, and non-pharmacologic interventions (e.g., redirection and reassurance) is crucial.
- Creativity and Adaptability: The ability to design engaging, age-appropriate activities that are tailored to individual needs and can be adapted to varying cognitive and physical abilities.
- Communication Skills: Excellent verbal and written communication skills to interact effectively with residents, families, and interdisciplinary health care teams.
- Organizational and Leadership Skills: Needed for planning monthly calendars, documentation to include progress notes, MDS and etc. as well as volunteers.
- Certifications: Certification in dementia care and Activity Director Certified.
- 4-year Degree: Social Work or equivalent.
Core Responsibilities:
The responsibilities extend beyond just planning events: they focus on the whole well-being.
Person-Centered Planning: Assessing residents' interests, preferences, and past hobbies to create individualized and group activities that promote positive experiences.
Activity Implementation: Leading and facilitating a variety of therapeutic activities such as music and art therapy, gentle physical exercises, memory games and reminiscence activities, and education programs and social events.
Documentation and Collaboration: Maintain records of resident participation and progress and collaborating with other staff to align with resident's plan of care. Compile and record social histories to assist in understanding residents' backgrounds, family problems, resources, histories of illnesses, interests, etc. Record pertinent social data about medically-related personal and family problems in residents' medical records. Complete the social service portion of the Minimum Data Set.
Family Engagement: Communicating with families about their loved one's progress and providing guidance on meaningful engagement strategies.
Environment Safety: Ensuring activity spaces are safe, organized, and welcoming to minimize confusion and distress.
Technical Proficiency: Ability to use Electronic Health Records.
Soft Skills: High levels of empathy, patience, and crisis intervention skills to manage behavioral changes in residents.
