Schedule: Monday–Friday, Generally Day Hours with Rotating On-Call Responsibilities (Every 3rd Week)
Pay Rate: Up to $60.00 per Hour
Job Summary
MTK Healthcare INC is seeking a compassionate and dedicated Registered Nurse (RN) to join the team at Mosaic in Rockford, Illinois. This role focuses on providing holistic, person-centered care by assessing, coordinating, and monitoring the medical and psychiatric needs of individuals served. The RN will work collaboratively with healthcare providers, support staff, and interdisciplinary teams to ensure high-quality care while enjoying professional autonomy and a supportive work environment.
Key Responsibilities
- Assess, monitor, and coordinate the medical and psychiatric care of individuals served.
- Collaborate with physicians, community healthcare providers, nutrition services, and behavioral health specialists to develop and implement individualized care plans.
- Coordinate healthcare appointments and ensure timely follow-up care.
- Train, educate, and delegate medication administration and nursing tasks to the Direct Support Team in accordance with state regulations and company policies.
- Maintain, review, and audit medical records to ensure accurate documentation and compliance.
- Monitor ongoing health concerns and communicate changes to the appropriate healthcare providers.
- Participate in the on-call rotation every third week, primarily providing telephone support for emergencies.
- Build positive relationships with individuals served, families, healthcare professionals, and support staff.
- Ensure compliance with all nursing standards, state regulations, and organizational policies.
Minimum Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of one (1) year of clinical Registered Nurse experience within the last five years.
- Excellent assessment, documentation, communication, and organizational skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Strong commitment to providing compassionate, person-centered care.
Physical Requirements
- Ability to stand, walk, bend, and reach frequently throughout the workday.
- Occasional kneeling, climbing, stooping, crouching, and balancing.
- Ability to lift up to 25 lbs independently.
- Ability to push or pull up to 40 lbs of force.
- Ability to safely assist with transfers up to 50 lbs.
