Responsibilities:
Administer medications, treatments, and therapies as prescribed by physicians
Monitor and document patient vital signs, conditions, and progress
Develop and implement individualized care plans in collaboration with the multidisciplinary team
Provide education and support to patients and families on disease management, wound care, and other health-related topics
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Collaborate with other healthcare professionals to ensure seamless patient care
Requirements:
Current Registered Nurse (RN) license in the state of Pennsylvania
Minimum 1 year of experience as a Registered Nurse (RN) in a long-term care or rehabilitation setting
Strong clinical skills and knowledge of nursing practices and protocols
