JOB TITLE: Assertive Community Treatment (ACT) Lead Therapist
Education/Experience: Master’s degree in social work, counseling, or psychiatric nursing from an accredited graduate program plus one year of experience in providing adult services in a mental health setting.
Required Knowledge, Ability, and Skill: Requires a working knowledge of community mental health principles and practice, ability to relate effectively to seriously mentally ill adults and skill in development and implementation of treatment plans.
PRIMARY DUTIES AND RESPONSIBILITIES:
Provides direct clinical services, including but not limited to, intake assessments, individual, family, and group therapy/counseling, medication monitoring, crisis intervention, daily living skills training, and family support/education. Services are provided in the community setting.
Provides consultation to community service providers, including pharmacies, schools, emergency rooms, social service agencies, and allied health care personnel.
Provides indirect services in the form of treatment plan development/authorization, timely documentation of services rendered, case referral, case coordination, and efficient management of an assigned caseload.
Participates constructively in performance improvement/quality assurance activities, staff conferences, and in-service training.
Assess problems and symptoms; develop a diagnostic impression according to DSM-V-TR codes and criteria.
