Signature Albany Hospice is seeking a skilled and compassionate Registered Nurse (RN) Case Manager to provide high-quality Hospice care to patients in the comfort of their homes.
This full-time role is ideal for an RN with experience in Hospice, Home Health, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.
What You’ll Do
Patient Care & Case Management
-
Perform comprehensive nursing assessments and develop individualized plans of care

Coordinate and manage hospice services in collaboration with physicians and care teams

Provide skilled nursing care, treatments, and patient education
-
Monitor patient progress and communicate changes in condition to physicians

Help ensure high-quality, compassionate hospice services are delivered consistently.
Care Coordination & Documentation
-
Maintain accurate, timely clinical documentation and patient records
-
Communicate with healthcare providers and community resources
-
Ensure access to necessary medical equipment and services
Leadership & Supervision
-
Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs
-
Provide guidance to ensure quality care delivery and compliance
Additional Responsibilities
-
Participate in IDG, quality assurance and performance improvement programs
-
Attend training and in-service education
-
Participate in on-call rotation as required
✅ What You’ll Need
-
Graduate of an accredited nursing program
-
Active Registered Nurse (RN) license – Oregon
-
Minimum 2 years nursing experience (hospice, home health or public health preferred)
-
Current CPR certification
-
Valid driver’s license and reliable, insured vehicle
