Duties
- Conduct outreach to community members to assess their health needs and connect them with appropriate services
- Provide education on topics such as nutrition, medication management, addiction counseling, and preventive health measures
- Assist individuals with disabilities or chronic conditions in navigating healthcare systems and accessing resources like Medicare
- Use motivational interviewing techniques to encourage positive behavioral changes and improve health outcomes
- Collect and document data related to community health trends, service utilization, and client progress
- Support public speaking engagements to raise awareness about health programs and available services
- Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans
Qualifications
- Prior experience or training in social work, public health, or related fields is highly desirable
- Strong understanding of medical terminology and healthcare systems
- Experience working with individuals with disabilities or those facing social barriers
- Excellent communication skills for teaching, public speaking, and motivational interviewing
- Ability to collect accurate data and maintain detailed records of interactions and services provided
- Knowledge of nutrition principles and addiction counseling techniques is a plus
- Demonstrated commitment to community service with a proactive attitude towards health promotion
- Bilingual in Spanish and or Portuguese preferred
