Gardens of Belden Village is seeking a compassionate and motivated Part-Time Speech Language Pathologist (SLP) to join our dynamic rehabilitation team. This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes communication, swallowing function, cognitive improvement, independence, and enhanced quality of life.
Evaluate residents and develop individualized speech therapy treatment plans based on clinical findings and functional needs.
Provide skilled speech-language therapy interventions addressing communication, cognition, swallowing, and other areas of functional performance.
Complete assessments and implement evidence-based treatment strategies to support resident goals.
Monitor resident progress and communicate findings with the interdisciplinary team.
Accurately document evaluations, treatments, progress notes, and other required clinical information in a timely manner.
Collaborate with nursing staff, physicians, families, and other therapy team members to support resident care goals and discharge planning.
Educate residents, families, and caregivers regarding treatment plans, safety strategies, and recommended interventions.
Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.
Participate in care conferences and team meetings as appropriate.
Ensure a safe and therapeutic treatment environment for all residents.
Graduate of an accredited Speech Language Pathology program required.
Current Speech Language Pathologist license in the State of Ohio in good standing.
Certificate of Clinical Competence (CCC) preferred.
CPR certification preferred.
Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.
Experience with Net Health documentation software preferred.
Experience with PointClickCare (PCC) preferred.
Strong communication, clinical, organization, and interpersonal skills.
Ability to work effectively as part of an interdisciplinary team.
