Job Summary:
We are seeking a skilled and compassionate Evening Shift LPN to join our dedicated team at Newburgh Health and Rehab. Responsibilities:
Provide direct care to patients, including administering medications, wound care, and vital signs monitoring
Develop and implement individualized care plans to meet patients' unique needs
Collaborate with the interdisciplinary team to ensure continuity of care and effective communication
Maintain accurate and up-to-date records and reports
Participate in infection control and safety protocols to ensure a clean and safe environment
Demonstrate compassion, empathy, and excellent communication skills in patient interactions
