Join a fast-growing mobile and virtual health team as a California-licensed Family or Adult Nurse Practitioner, delivering annual wellness visits and preventive care entirely from home on a Monday through Friday schedule, with no patient panel, no nights, and no on-call. What You'll Do
- Lead virtual annual wellness visits and health risk assessments for adult and geriatric Medicaid patients
- Perform medication reviews, cognitive screenings, and preventive care interventions
- Collaborate in real time with in-home clinical staff handling vitals and blood draws
- Coordinate referrals, community resources, and follow-up documentation
- Chart accurately in a modern EHR while managing multiple digital tools
- Build meaningful relationships with a diverse, often elderly patient population

Requirements
- Active, unrestricted California FNP or ANP license, board certified required
- Active Medi-Cal credentialing and enrollment already in place required, candidates not yet enrolled will not be considered
- Minimum 2 years of NP experience with at least 1 year of hands-on clinical work, hard requirement with no exceptions
- Primary care background with direct experience conducting annual wellness visits for Medicare or Medicaid populations
- Comfortable working from a laptop using video visit tools, HIPAA-secure messaging, and multiple systems simultaneously
- Reliable high-speed internet and a private, HIPAA-compliant home workspace
Preferred
- Multi-state APRN licensure
- Prior telehealth, mobile health, or startup clinical experience
- Comfort with the pace and ambiguity of an early-stage, high-growth team
