Responsibilities:
To ensure direct personal supervision of the individual to perform self-help activities encouraging skill development and follow-up services. To complete and maintain accurate documentation as required. Assistance with independent living skills, personal care, eating at times, personal hygiene, and activities of daily living. Secure or provide appropriate transportation for community activities. (Van is provided) Must have a valid Drivers License.
Applicant will also be responsible for documentation and house cleaning, if necessary.
Assist individuals with developing, implementing, monitoring and evaluation community inclusion activities as well as facilitating social interaction, socially valued behaviors and time activities. Supervise self-administrative of medications or assist individuals with medication and with other tasks delegated by an RN Nurse, if necessary.
Employer will be conducting Background Check.
Pass a pre-employment drug screen and random drug screens throughout employment.
High School Diploma / GED
Good Driving Record.
Required experience:
- working with intellectual disabilities individuals: over 6 months.
