Position Overview
The Licensed Occupational Therapist Wellness Consultant partners directly with individuals in congregate living homes to provide clinical consultation, wellness support, and care oversight that promotes independence, safety, and overall quality of life. This opportunity focuses on evaluating and enhancing functional status, activities of daily living (ADLs), independent living skills, fine motor coordination, strength, mobility, home safety, and the appropriate use of adaptive equipment.
Responsibilities also include developing individualized plans of care, collaborating with interdisciplinary care teams to ensure treatment plans are implemented effectively, and providing evidence-based interventions that improve function and overall well-being while monitoring progress and outcomes. The role also works closely with supported living staff, including nurses, direct support professionals, and house administrators, to promote the physical, cognitive, emotional, and social well-being of each individual through a coordinated, person-centered approach.
Required Qualifications
- Certified Occupational Therapist (OTR/L) with an active California license
- Motivated, engaged, and deeply compassionate
- Experience or strong interest working with developmentally delayed and physically impaired populations
- Solid clinical reasoning and clear, patient-centered communication
- Comfortable traveling within San Diego's greater North County area
- Ability to work independently in home and community settings, as well as collaboratively with care teams
- Able to understand and interpret complex written or verbal instructions
- Bilingual is a plus
Compliance Requirements
- OT License
- Live Scan/Criminal Background Check
- Physical Exam/TB Test
- Basic Life Support (BLS) certification
- Private provider insurance (Ex. HPSO)
- Valid driver's license
