Care Coordinator Motivational Interviewing Informed Wrap Preventive Services
Family Help Center is seeking full-time Care Coordinator/Case Planner for positions in our Motivational Interviewing Informed Wrap Preventive Services. Care Coordinators provide home-based services to assist youth and families with strength-based, individualized service planning.
Responsibilities:
- Manage a caseload of 10 families
- Conduct multiple monthly face to face home visits throughout Erie County
- Partner with Erie County Social Services and other community-based organizations
- Utilize an Evidenced Based Model to provide coordination to parents, children and youth
- Facilitate Team Meetings- assist families with developing a crisis plan, identifying needs, creating goals and building natural support system
- Timely completion of documentation between two computer systems
- Provide education and/or information to families to address case needs
- Make referrals to community-based services; monitor service provision
- Participate in mandatory trainings, specifically Motivational Interviewing Informed Wrap Model Credentialing

Minimum Requirements:
- Bachelor’s Degree in Social Work or related field with a minimum of one year direct service experience in the human services field (qualifying internship will be considered).
- Must possess valid NYS driver’s license, insurance and registration, and acceptable driving record.
- Preferred knowledge of trauma informed care and impacts of trauma on children and families.
- Clear all background checks and clearances.
- Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds.
- Must be able and willing to travel throughout Erie County during all seasons to provide services to clients.
- Ability to move about in a family’s home, climb stairs, work in an office setting with peers, sit at a desk, use copier/scanner/fax, phone, possess computer skills and operate technology (work issued cell phone and tablet/laptop and desktop).
- Ability to work independently with ample support, coaching and training.

Schedule
- 40 hours a week
- Monday through Friday; no weekends required
- Flexibility in daily schedule
Salary: Bachelors Degree $51,000 a year
Masters Degree $52,000 a year
Pay: $51,000.00 - $52,000.00 per year
