The School District of La Crosse is accepting applications for a 1.0 FTE Special Education Transition Coordinator for the District beginning in the 2026-2027 school year. Projected start date: August 11, 2026.
Wisconsin certification 1801 and/or 1810, 1811, or 1830 is required. Multiple certifications are preferred.
Prefer applicants possessing the following qualifications:
- Minimum of three years successful teaching experience in special education at the middle school or high school level.
- Master’s Degree in Special Education preferred.
Position responsibilities:
- Coordinate transition services to post-secondary education (college tours, program shadows, prep and set up for entrance testing, gather transition related data relevant to each college needs/requirements).
- Along with special education case managers, provide small group or individualized career exploration (WISCareers, surveys, inventories, etc) for students with disabilities.
- Provide instruction to students in the areas of: self-advocacy skills; job searching; awareness of the job market; completing applications; resume development; job interviewing; accessing financial aid for post-secondary education or training; life skills; budgeting/saving/investing; setting personal goals; independent living skills etc.
- Coordinate community experiences for students related to their transition goals.
- Develop mentorship sites in the community for job related experiences and coordinate student placements in these sites including transportation.
- Provide ongoing employer and student support through site visits or short-term job shadowing.
- Coordinate functional vocational evaluations for students and report results to students, case managers, and parents to help with planning.
- Coordinate transition efforts with other community agencies when appropriate (i.e. Department of Vocational Rehabilitation).
- Provide parent information opportunities on transition related topics.
- Provide instruction, information, and support for adult living options.
- Be the School District of La Crosse representative at monthly regional Transition Advisory Council (TAC) meeting.
- Attend other professional development opportunities related to improving transition services for students with disabilities/at-risk and present information learned to other department special education and related team members.
- Provide assistance and training with IEP and at-risk plan development in the area of transition and participate in state reporting of compliance related to transition requirements.
- Attend IEP meetings when requested by the case manager to provide input related to transition opportunities. Report student progress in the area of transition goals when appropriate.
- Collect longitudinal data on post high school outcomes for students with disabilities.
- Provide consultation to teachers and serve as a resource to parents/staff.
- Maintain all required records and submit reports to the Director of Student Services.
- Select and requisition instructional materials and supplies in accordance with district budget process.
- Supervise pupils in out-of-classroom activities during the assigned working day.
- Participate in program improvement and curriculum development programs as required.
- Complete other duties as assigned by supervisor.
