Physical Therapist — W-2 Employee:
PCP / Chiro/ PT Practice is a patient-focused healthcare practice committed to providing personalized, high-quality care. We are seeking a compassionate and skilled Physical Therapist to join our team on a part-time basis and help patients improve mobility, function, strength, and overall quality of life.
The Physical Therapist will evaluate and treat patients with a variety of musculoskeletal and functional conditions. The ideal candidate is clinically knowledgeable, patient-centered, and comfortable working collaboratively with physicians and other healthcare professionals.
Responsibilities:

Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

Provide therapeutic exercises, manual therapy, neuromuscular re-education, and other appropriate interventions.

Monitor patient progress and modify treatment plans as clinically indicated.

Educate patients on home exercise programs, injury prevention, and strategies for maintaining functional improvements.

Maintain accurate and timely clinical documentation.

Communicate effectively with physicians and other members of the healthcare team.

Provide a professional, compassionate, and positive patient experience.

Follow all applicable clinical, ethical, privacy, and regulatory standards.

Submit authorizations for Initial Evals/Re-Evals.
Qualifications:

Current New York State Physical Therapist license, or eligibility for licensure.

Current CAQH profile.

Strong clinical assessment and treatment skills.

Excellent communication and interpersonal skills.

Ability to work independently while functioning effectively as part of a multidisciplinary team.

Commitment to evidence-based, patient-centered care.

Prior outpatient or private-practice experience preferred.
Current participation with Medicare and major New York commercial insurance plans is strongly preferred. Please include a list of the insurance plans with which you currently participate. Our credentialing team will assist with adding the selected therapist to Front Street Medical’s group and service locations.
This is a W-2 employee position and is not an independent-contractor assignment.
