Advanced Home Health and Hospice of Las Vegas is hiring a full-time LPN (Home Health) to provide skilled nursing care to patients in their homes throughout the Southwest, Northwest, and North Las Vegas.

8 Hour Shifts

Field-based driving required across SW/NW/North Las Vegas (Primarily West Side of Las Vegas)

Provide skilled nursing visits in the home under the plan of care

Perform assessments, vitals, documentation, and patient/caregiver education
-
Coordinate with RN case managers and the interdisciplinary team
-
Communicate changes in patient condition and follow clinical protocols
-
Maintain accurate and timely charting per agency and regulatory standards
Requirements:
- Active Nevada (NV) LPN/LVN License (required)

Minimum 2 years of home health experience (required)

Valid driver’s license and reliable transportation (driving required)

Strong clinical skills and patient-centered care

Excellent communication and time management

Dependable, organized, and comfortable working independently in the field
