PURPOSE
This position is part of Vecino’s community health efforts to promote health and increase Vecino’s visibility in northeast Houston, Vecino HRSA-designated service area. This position delivers health literacy programming and provides health literacy and navigation services in northeast Houston/Harris County. The main purpose of these activities is to promote health and connect community members to Vecino. The CHW works collaboratively with other community-based organizations and public institutions in the northeast.
ESSENTIAL JOB DUTIES & RESPONSIBILITIES
1. Develop and implement weekly and monthly work plans to engage individuals and institutions in northeast Houston to provide health literacy and navigation services. A minimum of (AMO) 16 community visits (including events and presentations) should take place every week.
2. Impart health literacy programming to promote health in northeast Houston. Target: AMO one presentation per month.
3. Recruit new patients for Vecino’s health services. Target set annually based on prior year baseline.
4. Participate and represent Vecino at health/resource fairs and networking meetings in northeast Houston.
5. Help recruit participants for Vecino’s health literacy programming.
6. Use the asset-based methodology to organize and plan community health work.
7. Use a case management approach to provide navigation services in the community.
8. Maintain and manage supplies inventory for community health programming.
9. Identify and address barriers to care to ensure the effectiveness of community health programming.
10. Document and follow up with community requests for health literacy and navigation services.
11. Keep documentation and records of outreach events, contacts made, and people engaged in both health literacy and navigation services to issue weekly and monthly reports.
12. Help build collaborative efforts within the Vecino service area.
13. Participate in trainings and team activities such as patient appreciation efforts.
14. Adhere to policies and procedures outlined by Vecino.
15. Other duties as assigned by supervisor.
QUALIFICATIONS, EDUCATION, AND EXPERIENCE
1. Community Health Worker certification required; associate degree preferred
2. One or two years of minimum experience in community-based work
3. Bilingual English-Spanish; must be able to speak, read, and write fluently in both languages
4. Experience participating and organizing community events among diverse populations
5. Excellent organizational and interpersonal skills to establish effective working relationships
6. Tactful and respectful
7. Impeccable work ethic
REQUIREMENT
