In this role, you'll play a central part in ensuring our residents receive accurately assessed, properly documented, and well-coordinated care. You'll work closely with nursing staff, therapy, and administration to manage the MDS (Minimum Data Set) process and support quality outcomes for every resident we serve.
Coordinate and complete accurate, timely MDS assessments in compliance with federal and state regulations
Develop and maintain individualized care plans in collaboration with the interdisciplinary team
Conduct clinical and functional assessments of residents upon admission, quarterly, annually, and with significant change
Ensure accurate coding and submission of MDS data to CMS via the QIES/iQIES system
Monitor and track assessment reference dates and ensure compliance timelines are met
Collaborate with therapy, social services, dietary, and activities staff to reflect holistic resident needs
Assist in tracking and improving quality measures and facility quality indicators
Participate in care conferences and communicate assessment findings to residents and families
Support state and federal survey readiness related to MDS and care planning documentation
Current Pennsylvania RN or LPN license in good standing (required)
Knowledge of the RAI (Resident Assessment Instrument) process and MDS 3.0
Familiarity with PDPM (Patient-Driven Payment Model) preferred
Strong attention to detail and organizational skills
Excellent written and verbal communication skills
Comfortable working in electronic health record (EHR) systems
RAC-CT certification a plus
