About the Role:
As a Case Manager/Care Coordinator, you’ll work directly with children and families to identify strengths, set goals, and connect them with the supports they need. You’ll facilitate Child and Family Team meetings, coordinate services, and collaborate with providers and natural supports. This role includes a mix of office, home, and community-based work.
