Job Overview
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in patient care and a passion for helping individuals achieve their maximum potential through therapeutic interventions. This role involves working with diverse populations geriatrics, and individuals with disabilities, in home health environments.
Responsibilities
- Assess patients' needs and develop individualized treatment plans to enhance their ability to perform daily activities (ADLs).
- Implement therapeutic interventions aimed at improving patients' physical, emotional, and cognitive functioning.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care and effective discharge planning.
- Provide education and support to patients and their families regarding care plans and rehabilitation goals.
- Maintain accurate documentation of patient progress and treatment outcomes in accordance with regulatory standards.
- Stay current with best practices in occupational therapy, including techniques for working with individuals with autism, acute care settings, and early intervention strategies.
- Participate in case management activities to coordinate care for patients across various healthcare settings.
Skills
- Proficiency in assisting with activities of daily living (ADLs) and developing care plans tailored to individual needs.
- Knowledge of medical terminology, anatomy, physiology, and the principles of occupational health.
- Experience working with diverse populations including geriatrics,individuals with disabilities, and those requiring post-acute care.
- Familiarity with various therapeutic settings such as inpatient units, skilled nursing facilities, and home health environments.
- Ability to provide speech therapy support when necessary and collaborate effectively within a multidisciplinary team.
- Strong communication skills to effectively educate patients and families about treatment processes and goals.
- Experience in case management is preferred along with a commitment to ongoing professional development in the field of occupational therapy.
