American Traveler is seeking an experienced RN Case Manager for an inpatient acute care position in California, requiring 5+ years of case management experience and a CA RN license.

Inpatient acute care hospital setting

Case Management unit focused on care coordination and care transitions throughout the acute care patient experience
-
Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to manage patient progression and transitions of care
-
Day shift, 5x8-hour shifts (8:00–16:30)

Every other weekend required

13-week contract

Active CA RN license required

Current BLS certification required

Minimum 5 years of RN Case Management experience required

Inpatient acute hospital Case Management experience is required

2 supervisor references obtained within the past year required for consideration

Copies of all certifications required for consideration

Responsible for care coordination and transition planning, including collaboration with interdisciplinary teams, payers, and community resources to ensure timely and appropriate patient transitions

Performs utilization management including level-of-care reviews, length-of-stay monitoring, and regulatory compliance (CMS, Joint Commission)

Screens 30-day readmissions and develops effective, realistic transition plans in collaboration with patients, families, and the care team

Identifies and addresses complex clinical and social situations to prevent unnecessary admissions and ensure appropriate resource utilization

Candidates must reside more than 50 miles from the facility to be eligible for the travel pay rate

Former employees of this health system must have been separated for at least 1 year before returning as a traveler

Only 1 holiday RTO request will be approved for the duration of the contract
