Position Summary:
We are seeking a Program Manager/Licensed Practical Nurse (LPN) to oversee caregiver staffing, client care coordination, and operational support for non-medical home care services. This role supports Ohio Medicaid Waiver programs through client assessments, caregiver supervision, recruitment, compliance oversight, and care planning activities. The ideal candidate combines strong clinical judgment with leadership, communication, and organizational skills to ensure high-quality care delivery, caregiver support, and regulatory compliance across field operations.
Key Responsibilities:
- Conduct client assessments, supervisory visits, and caregiver welfare checks to ensure quality care, compliance, and client safety.
- Collaborate with RNs and care teams to develop and update individualized service plans based on client needs and clinical observations.
- Recruit, interview, and onboard qualified caregivers to support ongoing staffing and client care needs.
- Provide field coaching and ongoing support to caregivers regarding ADLs, infection control, safety procedures, and client-specific care expectations.
- Maintain accurate documentation, client records, and recruitment activity in compliance with Medicaid, HIPAA, and state regulations.
- Build and maintain a strong pipeline of caregiver candidates through job boards, referrals, networking, and community outreach.
- Serve as a liaison between clients, families, caregivers, and internal leadership to ensure effective communication and continuity of care.
- Monitor caregiver performance, address concerns in the field, and support retention through ongoing engagement and follow-up.
- Participate in audits, quality assurance initiatives, staffing meetings, and operational planning activities.
- Travel within assigned territory to conduct client visits, caregiver supervision, recruiting activities, and community engagement efforts.
Qualifications:
- Active Licensed Practical Nurse (LPN) license in the state of Ohio (Required)
- CPR Certification (Required)
- Negative TB Test within the last 12 months (Required)
- BCI Fingerprinting upon acceptance of position (Required)
- Willingness to travel up to 100 miles, as needed to complete home visits (Required)
- Reliable transportation, valid driver’s license, and proof of auto insurance# (Required)
- Strong working knowledge of Ohio Medicaid Waiver programs
- Excellent documentation, communication, and organizational skills
- Ability to work independently in the field and manage multiple priorities

Base Pay: $55,000.00 - $65,000.00 per year
