The Social Services Manager will oversee and coordinate social services for residents within our Skilled Nursing and Long-Term Care facility.
- Assess residents' psychosocial, emotional, and discharge planning needs
- Develop and implement individualized social service interventions
- Coordinate discharge planning and transitional care services
- Assist residents and families with care planning and community resources
- Facilitate advance directive discussions and decision-making
- Coordinate Family Council meetings and family support initiatives
- Manage resident grievance and complaint processes
- Ensure compliance with state, federal, and company regulations
- Collaborate with interdisciplinary healthcare teams to enhance resident quality of life
- Advocate for resident rights, dignity, and emotional well-being
- Bachelor's Degree in:
- Social Work (BSW)
- Human Services
- Psychology
- Sociology
- Gerontology
- Rehabilitation Counseling
- Special Education or related field
- Minimum one (1) year of supervised social services experience in a healthcare setting
- Experience working with elderly residents or geriatric populations
- Strong communication, documentation, and organizational skills
- Licensed Social Worker preferred (LBSW, LSW, LMSW, LCSW, etc.)
- Prior Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, Nursing Home, or Healthcare Social Services experience strongly preferred

Requirements:
ENTRY QUALIFICATIONS· An individual with a minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and· One year supervised social work experience working directly with individuals in a health care setting.· Current Social Work license in the state, if applicable.
