Abitare 2.0 is rebuilding its care coordination model with a strong focus on structure, accountability, and consistent follow-up. We are seeking Lead Care Managers (LCMs) to support members through Enhanced Care Management (ECM) by ensuring no member falls through the cracks.
Position Overview:
The Lead Care Manager is responsible for managing a caseload of members, coordinating services, and ensuring consistent follow-up across care teams. This role requires strong organization, communication, and the ability to manage multiple responsibilities in a structured environment.

Key Responsibilities:
- Manage assigned member caseload and provide ongoing support
- Conduct outreach and maintain consistent follow-up
- Coordinate care across providers and internal teams
- Document activities accurately in systems (Monday.com, etc.)
- Track progress and escalate issues when needed
Qualifications:
- Experience in case management, care coordination, or healthcare preferred
- Strong organizational and time management skills
- Ability to manage multiple responsibilities and maintain follow-up
- Strong communication and professionalism

Job Type: Full-time
Pay: $26.00 - $28.00 per hour
