TheraLinks, LLC is a therapist-owned home health network founded right here in Ventura County by a Physical Therapist who understands what it’s truly like to be in the field — balancing patient care, documentation, travel time, and life outside of work.
We were created with one goal in mind: to take care of our local communities while supporting the clinicians who serve them.
We are seeking a compassionate and motivated Licensed Physical Therapist Assistant (PTA) to join our home health team on a PRN / independent contractor basis.
This role is ideal for clinicians who value true flexibility, meaningful one-on-one patient care, and a workflow designed to respect your time.

Your Role
- Provide skilled physical therapy interventions under the direction of a supervising Physical Therapist
- Carry out individualized plans of care focused on mobility, strength, balance, and functional independence
- Deliver hands-on care including therapeutic exercise, gait training, transfer training, and functional mobility
- Monitor patient progress and communicate changes clearly with the supervising PT
- Educate patients and caregivers on home exercise programs, safety strategies, and fall-prevention strategies
- Complete timely and accurate documentation using our easy-to-use electronic medical record system
- Support safe and effective discharge planning by reinforcing long-term strategies for success at home
Skills & Experience
- Strong foundation in anatomy, physiology, and physical therapy principles
- Comfortable working independently in the home setting while staying connected to the care team
- Experience with adult and geriatric populations preferred
- Organized, reliable, and confident managing field-based care
- Clear, compassionate communication style with patients and caregivers
Qualifications
- Active California Physical Therapist Assistant (PTA) License
- Current CPR/BLS Certification
- Valid Driver’s License and reliable transportation
- Ability to work independently and manage a flexible caseload
- Home health experience preferred — mentorship and guidance available
