Job Description
- Provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
- Work closely with management to ensure we are providing excellent resident care.
- Delegate care aides to pass medications
- Assist in assessments of new residents as needed
- Do weekly med cart audits
- Review of medication administration records
- Provide health monitoring and follow up for each resident upon an envent, incident, or change in condition
- Assist in investigations
- Ensure weekly resident reviews are occurring with management
- Review of quarterly psychotropic medication
- Review of monthly psychotropic medication
- Review charting on a regular basis
- Assist with training care aides on nurse specific tasks.
- Prepare and administer medications as ordered by physician and perform other drug administration.
- Assist in care plan meetings as needed
- Update residents’ doctors and family as required, as needed.
- Assist in updating residents' assessments and ISP’s as needed
- Regular and continuous monitoring of wound care needs
- Communicate changes in patient clinical condition with physician, nursing supervisor and family
- Ensure that the community is in compliance and meets the standards of DHA 83 and Touchstone Senior Services.
- Assist in completing required record keeping upon residents' admission, discharge, transfer; chart and document per facility policy
Requirements
- Graduate from an accredited nursing program.
- Hold a Current State License as a Registered Nurse
- Able to communicate well in both verbal and written forms
- Have a minimum of 1 year experience as an R.N. (assisted living facility experience preferred but not required)
