Chartiers Center is a private, non-profit corporation funded by state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day and recreational services for adults with intellectual disabilities, homeless outreach and housing program.
As a Mobile Enhanced Psychiatric Rehabilitation Worker with Chartiers Center, you will work in the home and community with our program participants to help them to live full and productive lives. Job Duties include:
Meet weekly with a caseload of clients in their home or community to teach them needed skills such as independent living skills, social skills, and job search skills, and help connect them with supports such as needed living resources and social opportunities
Maintain a weekly schedule where the majority of your work time is spent in face-to-face client meetings
Thoroughly document client meetings to reflect work completed and interventions delivered, as well as client’s response to service and any client concerns
Provide on-call on a rotating basis.
Available to work a flexible schedule
Specialist Only- Complete Comprehensive Assessments and Individual Rehabilitation Plans for assigned caseload.
