PIE Management is seeking dedicated and compassionate Social Workers to provide comprehensive case management and support services to older adults and individuals with disabilities throughout the Detroit area.
The ideal candidate will possess a Bachelor of Social Work (BSW) or a related degree and demonstrate experience in case management, care coordination, community resource navigation, and person-centered planning. This position offers the opportunity to make a meaningful impact by helping vulnerable populations maintain independence, improve quality of life, and access essential services.
Conduct comprehensive biopsychosocial assessments in client homes and community settings.
Evaluate social, emotional, environmental, and healthcare needs.
Develop, implement, and monitor individualized care plans based on client goals and changing circumstances.
Conduct screenings related to home safety, social isolation, physical well-being, and long-term care needs.
Manage a caseload of approximately 50–70 clients.
Conduct regular follow-up through home visits, phone calls, and care coordination activities.
Connect clients with community resources including Medicaid, Medicare, Meals on Wheels, transportation assistance, housing resources, and supportive services.
Advocate for clients and caregivers to ensure access to appropriate services and support systems.
Provide supportive counseling, motivational coaching, crisis intervention, and behavioral health support.
Facilitate individual and group counseling sessions as appropriate.
Support individuals experiencing dementia, Alzheimer's disease, PTSD, developmental disabilities, autism spectrum disorders, and other behavioral health challenges.
Maintain accurate, timely, and HIPAA-compliant case documentation.
Complete assessments, care plans, service authorizations, and progress notes within established timelines.
Utilize Electronic Medical Record (EMR) systems and case management software.
Participate in interdisciplinary care planning and clinical review processes.
Work closely with healthcare providers, caregivers, community organizations, and social service agencies.
Participate in team meetings, case reviews, and program improvement initiatives.
Support research and quality improvement activities related to aging services and behavioral health programs.
One (1) to two (2) years of experience in human services, aging services, community health, healthcare, or related social service settings preferred.
Experience in case management, community resource coordination, and care planning.
Knowledge of aging services, senior care, assisted living, hospice care, and home- and community-based services.
Familiarity with behavioral health practices including crisis intervention and motivational interviewing.
Experience working with individuals diagnosed with dementia, Alzheimer's disease, PTSD, developmental disabilities, or autism spectrum disorders.
Knowledge of HIPAA regulations, medical documentation standards, and healthcare compliance requirements.
Proficiency with EMR systems, electronic case management platforms, and Microsoft Office applications.
Strong organizational, communication, and interpersonal skills.
Ability to work independently while collaborating effectively within multidisciplinary teams.
