Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations.
- Manage complex clinical procedures such as wound care, tube feeding, ventilator management, and sterile techniques in accordance with best practices.
- Document all patient interactions meticulously within electronic health record (EHR) systems, ensuring compliance with HIPAA and confidentiality policies.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs such as dementia, Alzheimer's disease, developmental disabilities, or acute conditions.

Skills
- Proven experience working with geriatric patients in settings such as skilled nursing facilities or long-term care environments.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding standards (ICD-9/10).
- Skilled in using EMR/EHR systems
- Proficiency in performing advanced procedures like venipuncture, phlebotomy, suturing, airway management, and specimen collection/processing.
- Experience working with individuals with disabilities or behavioral health needs; familiarity with dementia or memory care is highly valued.
- Ability to communicate effectively with patients of all ages while demonstrating compassion and professionalism.
- Knowledge of infection control practices and long-term care regulations to ensure safe patient environments.
